Terms and Conditions
Accuracy: The details set out in this Membership Agreement are true and correct. If they change, I acknowledge that I am required to notify Faultline Barbell Limited of the changes in writing as soon as possible after they occur. If the details provided are not true or are misleading, I acknowledge my membership may be terminated at the discretion of Faultline Barbell Limited.
Bound by rules: I acknowledge that entry into this agreement means I will be bound by the constitutions, regulations, policies, manuals, guidelines and reasonable directions that Faultline Barbell Limited may make and amend from time to time ("Rules). I acknowledge that my failure to comply with any Rules may, in Faultline Barbell Limited’s sole discretion, result in termination of this membership agreement and revocation of my right to use Faultline Barbell Limited’s facilities.
Nature of membership: I acknowledge that my membership is based on my entitlement to use Faultline Barbell Limited’s facilities, not on actual use. If I do not redeem this entitlement, I acknowledge that I am still obliged to observe my responsibilities under this agreement, including payment of fees, during the period of non-redemption.
Accept risk of powerlifting: I have voluntarily accepted and acknowledge the inherent risk of danger and injury involved in powerlifting.
No liability: I will not hold Faultline Barbell Limited or its respective officers responsible for any claims, losses, expenses or costs (including legal costs) which may arise from or in connection with my membership and/or participation in any activity authorised or recognised by Faultline Barbell Limited.
Indemnity: I indemnify Faultline Barbell Limited from all claims, losses and expenses and costs (including legal costs) suffered or incurred at any time because of, or resulting directly or indirectly from, my failure to observe the constitutions, regulations, policies, manuals, guidelines and reasonable directions of Faultline Barbell Limited.
Medical Declaration: I am medically and physically fit and do not suffer from any injury, disease or condition, either physical or mental, that would affect my ability to safely participate in any authorised or recognised activities of Faultline Barbell Limited. If I am unsure about this declaration or suffer from such a condition I have either listed it on this form or I am aware that I must bring it to the attention of Faultline Barbell Limited.
Privacy: I agree that Faultline Barbell Limited can collect, hold, use and disclose my personal information as provided on this Form (and any updated or additional personal information Faultline Barbell Limited obtains from me including any photo or other record of my image) for the purposes of:
a. Processing my application for membership, compiling a register of members, and for requesting me to renew if my membership lapses;
b. Putting my name and contact information on Faultline Barbell Limited’s membership list for use by other members of Faultline Barbell Limited;
c. Publishing any of my powerlifting results on any of Faultline Barbell Limited’s websites;
d. Providing me with information and activities relating to Faultline Barbell Limited matters;
e. Including my photograph or other imagery on Faultline Barbell Limited’s website, in newsletters, annual reports, or similar official publications;
f. Enabling Faultline Barbell Limited to contact me with information about the products and services of Faultline Barbell Limited’s sponsors or funders (unless I have opted out of receiving such information on the Membership Form);
g. Enabling Faultline Barbell Limited to comply with any statute, regulation, by-law or other regulatory instrument that requires collection or disclosure of personal information;
h. Retaining the information provided on this form if my membership lapses (as an inactive member) for a maximum period of three years for the above purposes; and
i. Any other purpose I agree to in writing.
Use, Security and Access: I understand that my personal information will only be used for the purposes listed and in accordance with Faultline Barbell Limited regulations, and that:
a. My personal information will be held securely;
b. I will have access to my personal information under the Privacy Act;
c. My personal information will be corrected upon request.
Continued Membership: I understand that upon payment of my membership fee(s), I will become a member of Faultline Barbell Limited and that by paying such fee(s) by the due date(s), I will continue to be a member of Faultline Barbell Limited for the duration of my Membership as specified on this form and in accordance with Faultline Barbell Limited Constitution, unless I resign or my membership is terminated.
Pauses in Membership: Memberships cannot be paused or put on hold within the first 6 months of membership with Faultline Barbell Limited. Thereafter, members are entitled to 2 periods per year of no longer than 4 weeks each where payment may cease. This is at the discretion of the member but must give notice of start and end dates.
Interpretation: Every reference to “I” and “my” in this document is a reference to the party entering into this agreement with Faultline Barbell Limited.
Notice Period of Termination: Provide no less than 4 (four) weeks written notice of termination of membership.
Performance Enhancing Drugs (PEDs): Faultline Barbell Limited and its officers promote drug free sport. Any use or promoting the use of PEDs will result in the immediate termination of membership.
Student Membership: Student membership rates shall apply only to those actively studying at a secondary school or tertiary institute for the duration of such study. Faultline Barbell Limited may ask for current student ID to be produced from time to time and such ID must be produced to qualify for student membership rates. Standard membership rates shall apply upon the conclusion of study. Once study has finished you must notify Faultine Barbell Limited within 2 months. Standard membership rates shall apply thereforth.
